The following forms are required to complete registration.
Registration items to be returned to office:

- Student Info (New Students)/Household Changes Form (Returning Students): The office is required to have accurate contact/household information for each child. This includes an accurate mailing address and current phone number for parent/guardian. Returning Student Form >>New Student Form
- Current Medical and Emergency Information: This include consent for treatment in case of emergency. A current signed copy is kept at the office.
- Student│Parent│Teacher Compact: Required for all schools receiving Title 1 funds.
- Internet Access Conduct Agreement
- Chromebook Agreement
- Reporting on Race/Ethnicity: Required by OPI
- Authorization to Carry and Self Administer Medication: for prescription medication only.
- Student Directory Information Notification: Return this form if you DO NOT want your child’s photo or name to appear in the yearbook, music or sports programs, local newspaper, tournament programs, etc.
